I am assuming you plan to send people offshore so that they can get their surgery/treatments done at a lower cost and without the long delays.

The first step need to tie up with some of the promitnent hospitals in India/anu other country which does that. (Apollo is one such hospital which comes to my mind.)

Reach the targetted customer base (people with no insurance/ high deductible insurance) who are in need ot elective surgery.

Take it from there.
